Applicant Responsibilities

All documents must be turned in at one time. After submitting the online application, applicant has 15 days from the submission date to provide CMHA written documentation for all members listed on the application the following verification: birth certificates, social security cards, drivers license or state ID, all income sources (i.e. employment verification or 6 pay stubs, Social Security Administration Awards Letter, pension document, Food Stamp (SNAP) print out and/or any cash assistance received from OJFS, etc.)

Applicants are required to inform CMHA, in writing, of changes in address or family composition.

Failure to provide information including failure to submit the written documents within the 15 days from the online application date to the CMHA or respond to mailings may result in the applicant’s name being removed from the waiting list. You may return this paperwork in one of our 3 dropbox locations.
